Requirements
- Application in EAP-I form
- Ten latest passport size photographs (full frontal view of face against light background)
- The damaged passport in original and a photocopy thereof
- Copy and original of the Identity Card/Labour Card/Health Card/Driving Licence to establish identity and nationality (The original will be returned after verification)
- A Sworn Affidavit in the prescribed format explaining -
a) The circumstances under which the passport was damaged;
b) Giving details of journey(ies) performed on the lost passport;
c) Concessions like TR benefits/FT withdrawals, gold/silver imports availed of on the lost passport; and
d) Places of stay and work in UAE since arrival
- Submit in person to establish physical identity
Time Taken
A duplicate passport with residual validity will be issued after the passport particulars have been verified and confirmed by the Passport Issuing Authorities or three months, whichever is earlier. In case of emergency, a limited validity six-month passport will be issued upon production of documentary proof to that effect. Where a limited validity passport is issued, full residual validity will be restored upon verification and confirmation of passport particulars. In such cases the Consular Officer may be contacted for advice.
